TCHATT Referral Steps
TCHATT is NOT a Crisis Response. Please do not refer students who are in an active crisis.
1. Discuss the program with the parent/guardian. Give them the online link to the TCHATT program consent forms or print out paper copies and have them complete them. Online (English | Spanish) or Paper (English | Spanish)
If the parent/guardian completes the TCHATT program consent forms, they will sent directly to us. Remind the parent/guardian that they will need to verify their email with the online consent forms.
Paper consent forms can be attached to the student's referral in Trayt
2. Sign into Trayt and submit the referral.
The TCHATT Clinician or care assistant will contact you if they are having trouble reaching the family or follow-up on missing or incomplete consent forms.
