It is important to start planning and preparing for health care transition as early as possible. MyChart and the Children’s Health app
Technology can help you manage your health care. You can see your Children’s Health℠ medical history through MyChart, an online medical records tool used by many health care systems. Even if you move or get care elsewhere, you will always have access to your Children’s Health records in MyChart. You can log in to MyChart on our patient portal site from a browser, or use the Children’s Health app or MyChart app on your mobile phone.
- Starting at age 14, a patient can have their own MyChart account. Ask your parent or guardian to request a minor account for you to become familiar with MyChart before you turn 18.
- At age 18, a parent’s access to their child’s MyChart account will end. If the parent will have medical power of attorney after the patient turns 18, contact the HIM department and provide a copy of legal medical decision-making documents for continued MyChart access.
- At age 18, a patient can share their Children’s Health MyChart record by choosing to give access to friends and family.

Get involved
Interested in sharing your ideas and perspectives on improving care at Children’s Health? Join the Youth and Young Adult Board (YAB).
Through the Family Advisor Network (FAN), Children’s Health partners with current and former patients ages 13-21.
Members of the board will provide their insight, experience, and guidance on the care of teens and young adults across Children’s Health. Meetings are held on a monthly to quarterly basis and you can attend both in person and virtually.
