Pediatric remote patient monitoring (RPM)
The remote patient monitoring (RPM) program allows clinical staff at Children’s Health℠ to monitor patients’ health statuses and progress via secure mobile technologies including one-touch video conferencing for virtual visits from the comfort of a patient’s home. The program aids in facilitating more advanced, efficient and patient-focused care management, while enhancing patient compliance.

By reducing the number trips to the hospital for follow-up visits, the RPM program improves health equity for families that must travel long distances to reach our specialists. With 2,844 participating patients to date; here are some of the specialties available for remote patient monitoring.
ECM - navigator-medically complex
This program allows the Enterprise Care Management’s navigator team to monitor/manage medically complex patients outside of traditional clinic visits, such as in the home or in a remote area; via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by reduction of ER visits and readmissions due to exacerbation of chronic illnesses. (i.e. educations, logic-based questionnaires, virtual visits).
ACO ED transitions
This program allows the Enterprise Care Management’s care management team to monitor/manage patients that have recently been discharged from Children’s Health emergency departments (Dallas and Plano); via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by mitigating the risk of readmissions by providing evidence-based, virtual interventions. (i.e. educations, logic-based questionnaires, virtual visits).
ACO IP transitions
This program allows the Enterprise Care Management’s care management team to monitor/manage patients that have recently been discharged from Children’s Health inpatient departments (Dallas and Plano); via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by mitigating the risk of readmissions by providing evidence-based, virtual interventions. (i.e. educations, logic-based questionnaires, virtual visits).
Complex care
This program allows the Enterprise Care Management’s complex care team to monitor/manage medically complex patients outside of traditional clinic visits, such as in the home or in a remote area; via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by reduction of ER visits and re-admissions due to exacerbation of chronic illnesses. (i.e. educations, logic based questionnaires, virtual visits).
Pulmonary
This program allows Children’s Health – Our Children’s House pulmonary care team to remotely monitor/manage patients that have recently discharged from the facility accompanied by a trach/vent; via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by mitigating the risk of readmissions by providing evidence-based, virtual interventions (i.e. educations, logic-based questionnaires, virtual visits).
Center for Cancer and Blood Disorders (CCBD)
This program allows Children’s Health’s Center for Cancer and Blood Disorders team to remotely empower patients that have recently been diagnosed with cancer; via their cellular phone. Program’s aim is to provide electronic resources and education for the parents of newly diagnosed patients. These resources may include but are not limited to general education, daily care instructions, and electronic ER “fast pass”. Program’s aim is to eliminate the need for a physical binder of information/education as a consolidated resource and improve ER throughput and treatment.
Post bariatric surgery
This program allows Children’s Health’s post-surgical team to remotely empower patients that have recently had bariatric surgery; via their cellular phone. Program’s aim is to increase access to care and decrease healthcare delivery costs by reduction of post-surgical readmissions due to noncompliance. (i.e. educations, logic-based questionnaires, virtual visits).
